They evaluated 24 patients with "diverse" disease processes. The subjects were assigned randomly to three groups. The treatment for Group I was preoxygenation/hyperinflation and suction with and without oxygen insufflation. 15 L with 100% oxygen followed by 15 sec of suction and repeated three times. 43 and four of the six subjects on PEEP. The treatment for Group II was a comparison of preoxygenation/hyperinflation and suction to oxygen insufflation/suction alone.

The difficulty in drawing conclusions regarding the effectiveness of oxygen insufflation to prevent suction-induced hypoxemia relates to the variability in the amount of oxygen insufflated 4 to 15 L/min and the diversity of subjects tested. The latter ranged from healthy anesthetized subjects to subjects with severe respiratory failure. Hyperoxygenation/Hyperventilation Downes, Wilson, and Goodson (1961) examined the effect of 15 sec of hyperventilation with oxygen in preventing arterial oxygen desaturation during apnea and apnea with ETS in 11 anesthetized adult patients undergoing pulmonary resection with the pleura closed or open.

Pulmonary compliance could be restored with inflation pressures of 25 to 30 cm H2O, thus bringing attention to the magnitude of atelectasis with suctioning. The extension of their findings to the premature infant is limited because of the relative maturity and healthy state of the respiratory system in the newborns in their study as compared to the premature infant.
